While “The Fate of Ophelia” music video leaned all the way into Taylor Swift’s showgirl era, “Patient Zero” is a treasure trove of refined capsule pieces.
On September 27, Swift aptly debuted her latest music video at the 2026 VMAs, where she also took home video of the year award for “The Fate of Ophelia,” as well as the inaugural Artist-Director Honor. “Patient Zero” marks the 18th video Swift has self-written and directed, as she pointed out in her acceptance speech. “I’ve gotta tell you, though, that the main reason why I love writing and directing music videos so much is because you, the fans, make it so much fun,” Swift said. “It’s so rewarding because you actually care about what we’re making.”
Yes, Swifties certainly care about each of frame of Swift’s new music video, whether picking apart every moment for clues and Easter Eggs or hunting down every item from her and co-star Dakota Johnson’s shared wardrobe.
If “Opalite” is a retro rom-com, “Patient Zero” could be considered a thriller, with Swift haunting a woman (Johnson) in the throws of a dangerously toxic relationship with a narcissist portrayed by Colin Farrell. By the end, we discover Swift is Farrell’s first wife, a deceased heiress who left behind billions to her presumed murderer. (One has to wonder if there’s potential for a “No Body, No Crime” music video sequel where Farrell meets his death at the hands of the Haim sisters.)

Throughout the video, Swift uses her and Johnson’s matching wardrobe as a visual metaphor for their shared experience—a rather expensive wardrobe made up of the sumptuous knits, luxe silks, and opulent evening wear favored by women of a certain tax bracket. It’s only when we see Swift’s own tragic story unfold that she wears her own looks, including the black silk Staud dress featured in one of the “Patient Zero” CD covers.
